Jacobs & Modaber is a full-service primary care practice, offering a complete spectrum of healthcare services including internal medicine. Healthcare can be complex and costly, but our goal is to simplify and control the cost of your healthcare through a concept called the Patient Centered Medical Home (PCMH).

Founded in 1963, Jacobs & Modaber Primary Care provides high quality, patient centered care to the members of the Las Vegas community.  Our physicians have experience in family and internal medicine.

Our main office is located at 2870 South Maryland Parkway, Suite 300 in Las Vegas, Nevada. Our normal office hours are Monday through Thursday, 8:30 a.m. to 6:00 p.m., and Friday, 8:30 a.m. to 5:00 p.m.

Our second location, Jacobs & Modaber West, is located at 2950 South Rainbow Boulevard, Las Vegas, Nevada. Normal office hours are Monday, Tuesday, Thursday and Friday, 8:30 a.m. to 5:00 p.m., and Wednesday, 8:30 a.m. to 6:00 p.m.
